| Purpose: Hospice is a medically directed, interdisciplinary program of palliative services for terminally ill persons and their families. Private, non-profit Hospice.
Services: Hospice emphasizes pain and symptom control wherever the patient resides by a team of professionals, including nurses, social workers, chaplain, nurse’s aides and volunteers, with consultation with physicians. Expert emotional and spiritual support and end of life care offered. 24 Hr. On-Call availability.

| ELIGIBILITY: | Any resident of Miami County and surrounding areas w-ho is terminally ill.
|
| DOCUMENTS NEEDED: | None

| RESTRICTIONS: | If on skilled care in a nursing home.
|
| HOW TO USE: | Call the office.
|
| FEE POLICY: | Insurance companies, including Medicare/Medicaid (as appropriate), will be billed directly. Eligible beneficiaries will be admitted without regard to ability to pay.
