Located conveniently between Piqua and Troy on spacious
landscaped grounds, this long-term care facility offers residents
and families individualized choices for life enhancement.
Services:
Include skilled level of care for short-term rehabilitation needs and an extensive restorative nursing program, 24-hour RN/LPN staff, hospice care, daily social events, discharge planning, chaplaincy service, home-style cooked meals. Features include: largest semi-private rooms in this area, individual windows for every resident, and large closet with storage! Included at no additional charge laundry service. Information on telephone service in each room is available on request.
Specialized Therapy Programs:
Include: physical, occupational, speech, and psychological under medical direction.
Other Medical Services:
Include: respiratory, podiatry, optometry, audiology and dental.
Freedom Court:
A specialized 25-bed unit where the unique needs of residents with Alzheimer's Disease and related memory disorders can best be met by a professional staff who have specific training to conduct specialized programming designed to meet each resident's needs in a controlled environment.

| ELIGIBILITY: | Persons requiring 24-hour skilled, intermediate or Alzheimer care either on short-term or long-term basis, appropriate to long-term care placement via Passport and facility assessment. Short-term Respite care available also.
