	Housing assistance for adults with severe mental disabilities. CH has several housing options for SMD individuals. There is a short-term rental subsidy program, transitional housing program, a no interest loan program, and financial support for those that need 24-hour supervision in group homes or rest homes.
ELIGIBILITY:	Referrals are made by case managers of the Mental Health Centers and they determine whether an individual is SMD and eligible for the housing program.
DOCUMENTS NEEDED:	Income verification. Rent is based on the person's income.

HOW TO USE:	Obtain a referral from a Mental Health case worker.
