DOCUMENTS NEEDED:	Proof of need for exam/glasses, and proof of financial burden may be requested. The Lion's Club will do their own screening of applicants.
HOW TO USE:	Call or write to any of the Lion's Clubs listed above. In letter to Laura, Troy and West Milton Lion's Club explain what is needed (eye exam, glasses, etc.). Sight Committee shall investigate and report on applications. The decision on eligibility will be made then.
