Home Care Services as an alternative to nursing home care to persons who are income eligible and 60 years of age.
Level of Care Assessment when required for individuals who are seeking nursing home care.
Outpatient Counseling
Marital, family and individual counseling for individuals of all ages. Ohio Department of Mental Health certified facility.
Senior Outreach Volunteer Program
Volunteers provide errands, telephone support and friend visits to seniors in need.

PASSPORT/ASSISTED LIVING must be income eligible and 60 years of age or older.
CARE COORDINATION/CAREGIVER SUPPORT must be 60 years of age or older.

CARE COORDINATION/CAREGIVER SUPPORT: Funded by state and federal dollars—voluntary contribution.
Outpatient Counseling: Most insurances are accepted, as are Medicaid, Medicare and Worker's Compensation. A sliding fee scale is available.
