| Easter Seals Adult Day Services Piqua is a viable low cost way of keeping individuals who are in need of continuous care at home, in the community and with family and friends as long as possible. Easter Seals Adult Day Services Piqua is proud to be affiliated with Goodwill Easter Seals Miami Valley. Easter Seals Adult Day Services Piqua offers individuals the opportunity to interact with peers and develop meaningful friendships through planned activities, including community outings, exercise and intergenerational activities.
Breakfast lunch, snacks, and drinks are served daily, as well as transportation is available, if needed. We are staffed with a Director, RN, Activities Facilitator, Activity Assistants, Certified Nursing Assistants, and Volunteers.
Easter Seals Adult Day Services Piqua’s objectives are to prevent premature nursing home placement, prevent social isolation; provide respite care for families/care givers; promote good health practices; promote self-esteem and independent functioning.

| ELIGIBILITY: | Individuals who benefit from these services include seniors and older adults with health risks (Alzheimer’s disease/dementia, limited mobility, Parkinson’s disease, stroke) and younger adults with developmental disabilities. The following criteria describes individuals who would benefit from these services.
- A person living with relatives who are absent during the day and who needs minimal supervision or stimulation.
- A person living alone who needs proper nutrition and the fellowship of others.
- A person who needs health and nutrition monitoring.
- A family who needs relief from the care of an elderly person, who otherwise might be placed in a nursing home.

| DOCUMENTS NEEDED: | Clients will sign medical releases verifying free from communicable diseases.
|

| RESTRICTIONS: | We are unable to provide services for a person whose behavior is unsafe to self and/or others and bed-ridden individuals.
|
| HOW TO USE: | Call Easter Seals Adult Day Services Piqua to set up an appointment to visit their facilities.
|
| A daily fee is assessed for individuals who may not qualify for funding assistance. A client can choose to attend a full day or a half-day (under four hours). The fee will cover breakfast, lunch, and snacks. Additional fees are assessed for transportation. Easter Seals Adult Day Services Piqua is funded in part by Title III, Title XX, Passport, Alzheimer’s Respite Funding, Disability Waiver, and VA.

| OTHER SERVICES: | Transportation to the Meal Sites in Miami County is also available through Easter Seals Adult Day Services Piqua. For individuals needing transportation to a meal site for a hot, nutritious meal and socialization, can call the Piqua Center at 778-3680 and ask for the Transportation Coordinator who will assist you with your transportation needs. Lunch Meal Sites are located in Piqua and Troy.
