	Comprehensive Well-Child Care, including: Complete routine physical examinations Physical examinations for school, work permits, sports and daycare Developmental screenings Laboratory screenings Vision and hearing screenings Anticipatory guidance Parenting skills Immunizations
ELIGIBILITY:	Miami County area residents, birth to 20 years of age.
DOCUMENTS NEEDED:	Social Security card, Immunization records, income verification and Medical card.
RESTRICTIONS:	Infants, children and adolescents seen by appointment only.
HOW TO USE:	Call or stop by office to schedule an appointment.
FEE POLICY:	Charges are based on a sliding fee scale according to income and family size. Medical cards accepted. No one will be denied service due to inability to pay.
