ELIGIBILITY:	Clients must show proof of pregnancy (pregnancy testing on site).
DOCUMENTS NEEDED:	Income verification, identification (driver's license, state ID, social security card, medical card/insurance card).
RESTRICTIONS:	Physician visits by appointment only.

FEE POLICY:	Sliding fee scale based on gross income and household size. Accept Medicaid card, Amerigroup, Caresource, and Molina.
